Health Insurance Programs |

Community Health Center Network clinics enroll members with the following insurance: -
- Medi-Cal
- Healthy Families
- Healthy Kids
- Group Care
- Access for Infants and Mothers (AIM)
- CompleteCare (Medicare Advantage Special Needs Plan)
The above programs are offered by Alameda Alliance for Health (AAH) www.alamedaalliance.com, Blue Cross (BC) www.bluecrossca.com, and Health Net www.healthnet.com Health Maintenance Organizations (HMO).
| | Medi-Cal | Medi-Cal covers inpatient and outpatient services, pharmacy coverage, and vision coverage (through VSP). No co-payments or premiums are required. www.medi-cal.ca.gov | | | | Healthy Families | The Healthy Families Program offers low cost insurance for children and teens up to age 19. It provides health, dental and vision coverage to children whose families are between 100% and 200% of FPL (federal poverty level) and do not qualify for free Medi-Cal. There is a small monthly premium. www.healthyfamilies.ca.gov | | | | Healthy Kids | Healthy Kids offers low cost insurance for children and teens under the age of 19. This program provides medical, dental and vision coverage to children whose families are equal to or below 300% of FPL and do not qualify for no-cost Medi-Cal, Medicare, or Healthy Families. There is a monthly premium per child in a family plus applicable co-pays. www.alamedaalliance.com/eligibility_criteria.html | | | | Group Care | Group Care covers In-Home Supportive Services (IHSS) home care workers and is both an inpatient and outpatient coverage plan providing comprehensive medical, vision, and dental benefits. There is a low-cost monthly premium, plus the applicable co-pays. www.alamedaalliance.com/eligibility_criteria.html | | | | | Access for Infants and Mothers (AIM) | The AIM program offers low cost insurance for pregnant women during their pregnancy and for 60 days after delivery. The newborn is automatically eligible for enrollment into the Healthy Families Program. Pregnant women whose family income is too high for No-Cost Medi-Cal, or who have health insurance with a separate maternity deductible or co-payments over $500 may qualify. www.mrmib.ca.gov | | CompleteCare | CompleteCare is a Medicare Advantage Special Needs Plan created by the Alameda Alliance for Health.
